The LiUNA Midwest Region is proud to be offering several voluntary programs that are available to members and their families.  The programs provide choice to the members to build a benefits package that meets individual needs.  The programs
available are:

Short-Term Disability -  Provides members the ability to insure their paycheck/income if they are injured off-the-job orsick and unable to work. Maternity benefits are also included. 

Accident Indemnity Plan - Pays Cash Benefits to members if they or a family member is injured on or off-the-job and pays over & above any other insurance.  Benefits include hospitalization, office visits, physical therapy, ambulance, specific sum injury benefits, accidental death and dismemberment, etc…

Critical Illness Plan - Lump sum benefit of $20,000 for members and $10,000 for spouse if a critical illness occurs.  Covered illnesses are Cancer, Heart Attack, Stroke, Major Organ Transplant, Renal Failure, Carcinoma In Situ-25%, Bypass Surgery-25%.  Kids covered for Free at 25% of lump sum.

Dental &/OR Vision Plan - No networks-pick any dentist or eye doctor with no restrictions. There are no deductibles. Benefits stay the same year over year and rates are stable so price will not increase year over year based on utilization and demographics.

Retiree Health Plan - LaborSTAR program provides supplemental health insurance to retired members and spouses who are enrolled in Medicare Parts A & B.  Freedom to choose any doctor or hospital, claims are filed automatically, and during the annual open enrollment period, coverage is guaranteed, regardless of pre-existing health conditions.
